Goal: The goal of this promising practice is to increase physical activity in a diverse older adult population.

Impact: Participants in the Active Choices program showed significant increases in physical activity and a greater satisfaction with their body appearance and function. Participants of the program also showed decreases in their BMI.

Goal: The goal of the Place Matters initiative is to improve the health of participating communities by “addressing social conditions that lead to poor health” through “identifying the complex root causes of health disparities and defining strategies to address them.”

Impact: AC Place Matters has identified 10 best practices that could help other programs address social determinants of health: find and foster strong leadership, dedicate staff resources to the work, engage staff from across the local health department, contribute to building grassroots power, address root causes, partner with community organizations and leaders, partner with government institutions across sectors, work reactively and proactively, build capacity, and use tools that ensure a focus on health equity.

Goal: The goal of the Adolescent Community Reinforcement Approach is to help adolescents recover from alcohol and drug addiction.

Impact: Results from studies on this treatment program demonstrate that there can be superior engagement, retention, and short-term substance use outcomes for those in the A-CRA and ACC approaches compared to UCC. The ACC protocol can also result in significantly more patients linking to continuing care.

Goal: The goal of Adolescents Living Safely is to prevent HIV infection and AIDS among runaway adolescents.

Impact: Adolescents Living Safely changes youth sexual behavior to reduce transmission of HIV among runaways.

Goal: The goal of Aiming High is to increase the percentage of English learners scoring at or above proficient on California Standardized tests in language arts and math by three percentage points annually.

Goal: The goal of this program is to reduce alcohol misuse among adolescents.

Impact: Middle school students who receive the curriculum have increased knowledge about alcohol misuse when compared to a control group. Students who received programming in the 10th grade had significantly increased alcohol misuse prevention knowledge, decreased alcohol misuse, and increased refusal skills. During their first year of driving, students who received the curriculum were involved in fewer serious traffic or drug offenses than students in the control group.

Goal: The overall goals of Kid2Kid are to: enable children to become responsible, caring world citizens by assisting them to develop compassion and empathy for other children around the world; increase students' global awareness and cultural understanding, while strengthening social studies and geography content knowledge; provide support, acknowledgement, encouragement and hope for traumatized children worldwide; encourage peace in the world, promote non-violence, and initiate pro-social behavior, among all children worldwide; facilitate an on-line culture learning process, whereby participating Kid2Kid children will learn about themselves and others around the world on a deep, meaningful level; and encourage and improve Kid2Kid children's artistic and communicative skills as well as provide an outlet for emotional expression.

Goal: A.S.P.I.R.E aims to reduce teen tobacco use by helping current smokers to quit and preventing non-smokers from beginning to smoke.

Goal: ASSIST aims to develop a diverse group consisting of young people that will then influence their peers to defy the idea of smoking thus reducing the number of adolescent smokers and reducing its health effects.

Impact: A peer-led intervention reduced smoking among adolescents at a modest cost: the ASSIST program cost of £32 ($42 USD) (95% CI = £29.70–£33.80) per student. The incremental cost per student not smoking at 2 years was £1,500 ($1984 USD) (95% CI = £669–£9,947).

Impact: The Community Preventive Services Task Force (CPSTF) recommends school-based asthma self-management interventions to reduce hospitalizations and emergency room visits among children and adolescents with asthma. Evidence shows interventions are effective when delivered by trained school staff, nurses, and health educators in elementary, middle, and high schools serving diverse populations.
When implemented in schools in low-income or minority communities, interventions are likely to promote health equity.
